Changing Your Choices
United Supermarkets gives you an opportunity to change your benefit choices during annual enrollment each year.
Once you have made your enrollment choices, you generally cannot change them during the year. However, you may make certain changes if you have a qualified change in family status that affects your benefits. Typical family status changes include:
- Marriage or divorce
- Birth, adoption or placement for adoption of a child
- Death of a spouse or other eligible dependent
- Enrollment in (or loss of) state or federal medical coverage
- A change in your employment or your spouse’s employment status
- A child no longer qualifies as a dependent
- Active military service
- COBRA event
You must change your benefit elections online for any family status change as soon as possible and before 30 days have passed. If you wait longer than 30 days, no changes will be made to any coverage until the next annual enrollment – per IRS regulations. You will be required to send in documentation with proof of the event.
Your Special Enrollment Rights
If you are declining enrollment for yourself or your dependents (including your spouse) because of other health plan coverage, you may be able to enroll yourself and your dependents in our plan later. This could occur if you or your dependents lose eligibility for that other coverage (or if the employer stops contributing towards that other coverage).
However, you must enroll within 30 days after you or your dependents’ other coverage ends (or after the employer that sponsors that coverage stops contributing toward the coverage).
In addition, if you have a new dependent due to marriage, birth, adoption, or placement for adoption, you may be able to enroll yourself and your dependents. However, you must request enrollment within 30 days after the marriage, birth, adoption, or placement for adoption.
